儿童蓝痣误诊为色素痣临床分析
Clinical analysis of blue nevus misdiagnosed as pigmented nevus in children
摘要
Abstract
Objective To investigate the clinical characteristics of blue nevus in children,causes of misdiagnosis as pigmented nevus and corresponding preventive measures.Methods A retrospective analysis was conducted on the clinical data of 3 children with blue nevus who were misdiagnosed with pigmented nevus from January 2021 to December 2024.Results One patient in this group had a facial blue-black patch discovered at birth,which was diagnosed one year ago as pigmented nevus at another hospital.Several laser treatments were performed,but the results were not satisfactory.The misdiagnosis lasted one year.One patient had a black facial mass discovered at birth,which was left untreated.As the child grew and developed,the mass gradually increased in size.In the past month,itching symptoms had occurred,and repeated scratching led to ulceration and scabbing.The admission diagnosis was pigmented nevus,and the misdiagnosis lasted one week.A black patch on the upper lip was discovered two years ago.As the child grew and developed,its area increased significantly.The local hospital diagnosed it as pigmented nevus,but no treatment was given.The misdiagnosis period was two years.All three patients had similar clinical symptoms and were misdiagnosed as pigmented nevus at the initial diagnosis.After admission,all 3 cases underwent surgical resection of the lesion tissues after completing dermoscopy and other examinations.The postoperative suture sites healed smoothly,and the pathological results confirmed the diagnosis as blue nevus.Conclusion Blue nevus is mainly manifested as local black,blue-gray or iron-blue nodules,usually a single lesion,with a smooth or uneven surface and a multi-lobed shape,and clear boundaries.This disease has similar clinical manifestations to skin pigmented nevus,and the rate of clinical misdiagnosis is relatively high.In clinical practice,it is necessary to take advantage of clinical features,dermoscopy,and pathological examination for differentiation,which can improve the accuracy of diagnosis.关键词
