超敏C反应蛋白/白蛋白对腹膜透析相关性腹膜炎治疗效果的预测价值
Predictive value of high-sensitivity C-reactive protein/albumin for the therapeutic effect of peritoneal dialysis-associated peritonitis

Abstract
Objective To investigate the predictive value of high-sensitivity C-reactive protein/albumin(hs-CRP/Alb)for the therapeutic effect of peritoneal dialysis-associated peritonitis(PDAP).Methods A total of 96 patients with PDAP admitted from January 2017 to November 2023 were retrospectively selected as the research subjects.All patients received anti-infective treatment and were divided into the effective group(n=79)and the ineffective group(n=17)according to the therapeutic effect.The general data and serum levels of hs-CRP,Alb and hs-CRP/Alb were compared between the two groups.Logistic regression was used to analyze the influencing factors of ineffective PDAP treatment.The receiver operating characteristic(ROC)curve was used to analyze the predictive efficacy of each index for ineffective treatment.Results The dialysis age,interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)of the ineffective group was higher than that of the effective group(P<0.05).The levels of serum hs-CRP and hs-CRP/Alb in the ineffective group were higher than those in the effective group,while the level of Alb was lower than that in the effective group(P<0.05).Logistic regression analysis showed that after adjusting for dialysis age,IL-6,TNF-α and other related factors,serum hs-CRP(OR=1.719,95%CI:1.491,1.982),Alb(OR=0.598,95%CI:0.407,0.878),hs-CRP/Alb(OR=1.900,95%CI:1.760,2.052)were still independent influencing factors of ineffective treatment in PDAP patients(P<0.05).ROC curve analysis showed that the area under the curve of serum hs-CRP,Alb and hs-CRP/Alb levels in predicting ineffective treatment was 0.766,0.768 and 0.879,respectively.The predictive efficacy of hs-CRP/Alb was better than that of hs-CRP and Alb(Z=1.631,1.600,P=0.042,0.048).Conclusion hs-CRP/Alb is an independent influencing factor for the therapeutic effect of PDAP patients,and has a certain predictive value for ineffective treatment.It can be used as an auxiliary index to predict the therapeutic effect of PDAP and guide clinical decision-making.关键词
