基于Kano模型的急性加重期COPD患者呼吸困难恐惧缓解支持需求分析
Kano Model-based Analysis of Support Needs for Relieving Dyspnea-related Fear in AECOPD Pa-tients

Abstract
Objective To analyze the support needs for relieving dyspnea-related fear in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD),aiming to provide a reference for optimizing clinical nursing strategies.Methods A total of 337 AECOPD patients treated at Ganzhou City People's Hospital from January 2021 to May 2023 were selected using a random number table.A questionnaire was designed to evaluate the support needs for relieving dyspnea-related fear in AECOPD patients based on the Kano model and Delphi method.The Kano model was used to categorize need attributes.Better-Worse coefficient and two-dimensional matrix analyses were conducted to determine need attributes.The sensitivity(R)was calculated to identify the elements most in need of improvement and optimize strategies.Results Based on the frequency statistics,two-dimensional matrix analysis,and element selection line L of the Kano model,the need attributes were categorized,adjusted,and screened.As a result,all 11 needs were in the priority of improvement,comprising 6 one-dimensional requirements(54.55%),3 must-be requirements(27.27%),and 2 attractive requirements(18.18%).The highest R value was 0.357 and the lowest was 0.021."Medication"(R=0.357),"emergency safety management"(R=0.320)and"disease monitoring"(R=0.311)exhibited the highest sensitivity.Conclusion Among the support needs for relieving dyspnea-related fear in AECOPD patients,"medication","emergency safety management"and"disease monitoring"are given top priority.It is recommended to enhance patient education on medication,organize regular drills to improve patients'ability to manage dyspnea-related fear,and help them establish disease monitoring logs for regular assessment.关键词
