尿液谷胱甘肽、组胺检测在结核病诊断中的应用价值
Value of urine glutathione and histamine detection in the diagnosis of tuberculosis

Abstract
Objective To observe the changes of urinary glutathione(GSH)and histamine levels in tuberculosis patients and to investigate the clinical value of urine GSH and histamine detection in the diagnosis of tuberculosis.Methods Thirty tuberculosis patients were included in the tuberculosis group,thirty subjects with latent tuberculosis in-fection(LTBI)were selected in the LTBI group,and twenty healthy examinees were included in the healthy control group.The urinary GSH and histamine were detected in the three groups by enzyme-immunosorbent assay.The receiver operating characteristic(ROC)curve was plotted and the area under the curve(AUC)was calculated to evaluate the diagnostic effi-cacy of urine GSH and histamine for tuberculosis and LTBI.Results Urine GSH,histamine levels decreased successive-ly in the health control group,LTBI group,and tuberculosis group(all P<0.05).The AUC of urine GSH in distinguishing between LTBI subjects and healthy people was 0.687(95%CI:0.536-0.837),with the sensitivity of 0.667 and the spec-ificity of 0.700;AUC of urine histamine in distinguishing between LTBI subjects and healthy people was 0.743(95%CI:0.601-0.885),with the sensitivity of 0.967 and the specificity of 0.450;AUC of the two combined was 0.782(95%CI:0.655-0.90),with the sensitivity 0.867,and the specificity 0.600.The AUC of urine GSH in discriminating between tu-berculosis and LTBI was 0.878(95%CI:0.785-0.970),with a sensitivity of 0.867 and a specificity of 0.850;AUC of urine histamine for discriminating between tuberculosis and LTBI was 0.917(95%CI:0.841-0992),with a sensitivity of 0.683 and a specificity of 0.833;the combined detection of both markers yields an AUC of 0.960(95%CI:0.913-1.000),with the sensitivity of 0.933 and the specificity of 0.850.Conclusions There were differences in urine GSH and histamine levels among patients with tuberculosis,those with LTBI and healthy individuals.Urine GSH and histamine detection can help for the early diagnosis and differential diagnosis of tuberculosis.关键词
