基于3D打印模型调整腰椎前凸角预防腰椎融合术后下腰痛效果观察
Effect of 3D printed model-guided adjustment of lumbar lordosis angle on preventing low back pain after lumbar fusion surgery

Abstract
Objective To investigate the effect of 3D printed model-guided adjustment of lumbar lordosis angle(LL)on preventing low back pain after lumbar fusion surgery.Methods Patients who underwent lumbar posterior de-compression,bone grafting,fusion and internal fixation surgery were selected as research subjects.According to differ-ent screw placement methods during surgery,patients were divided into the observation group(25 cases)and control group(24 cases).In the observation group,screw placement and connecting rod bending were individually adjusted based on 3D lumbar model according to LL during surgery.In the control group,traditional surgical screw placement and empirical connecting rod bending were used.Surgical time,blood loss,fluoroscopy times,and length of hospital stay were recorded in both groups.LL and pelvic incidence-lumbar lordosis(PI-LL,the difference between pelvic inci-dence angle and lumbar lordosis angle)were measured preoperatively,3 days postoperatively,and 6 months postopera-tively.Visual analog scale(VAS)was used to assess low back pain preoperatively,3 days postoperatively,and 6 months postoperatively.Oswestry disability index(ODI)was used to assess the degree of low back pain dysfunction pre-operatively and 6 months postoperatively.Postoperative complications were recorded in both groups.Results Com-pared with the control group,the observation group had shorter surgical time and fewer fluoroscopy times(both P<0.05).Compared with preoperative values,LL increased and PI-LL decreased in both groups at 3 days and 6 months postoperatively,and the observation group had greater LL and smaller PI-LL than the control group(both P<0.05).Compared with preoperative values,VAS and ODI scores decreased in both groups at 6 months postoperatively,and the observation group had lower scores than the control group(both P<0.05).The incidence of low back pain and postopera-tive complications in the observation group were lower than those in the control group(both P<0.05).Conclusion Ad-justing LL based on 3D printed model can reduce the incidence of low back pain after lumbar fusion surgery and promote lumbar function recovery.关键词
