Abstract
Objective:To investigate the impact of a closed-loop standardized positioning protocol on postural qualification rate in preterm infants in neonatal intensive care unit(NICU)and analyze its integrated effects on improving postural quality through multi-element synergy.Methods:A non-concurrent controlled study was conducted on 437 preterm infants admitted to NICU of the First Affiliated Hospital of Shantou University Medical College from June 2024 to January 2025.Participants were divided into control group(n=217,traditional positioning methods)and intervention group(n=220,standardized protocol including material standardization,systematic training,and dynamic quality control)for 3 months.The Infant Position Assessment Tool(IPAT)total scores,subscale scores,qualification rates,and nurses'theoretical test scores were compared.Results:The intervention group showed significantly higher IPAT total scores than controls(9.7±1.4 vs.8.0±2.0,t=-9.98,P<0.001),with qualification rate increasing from 44.2%to 79.1%(χ2=56.2,P<0.001).After Bonferroni correction,scores and qualification rates of 5 subscales(head,neck,shoulder,hand,hip)significantly improved(corrected P<0.008).Qualification rates by N2/N3-level nurses significantly increased(all P<0.001),and nurses'test scores improved from 40(20,60)to 100(100,100)(Z=-8.30,P<0.001).Conclusion:The closed-loop standardized positioning protocol significantly improves postural qualification rate in NICU preterm infants through synergistic effects of material standardization,systematic training,and dynamic quality control.关键词
