沂蒙老区患者对乡镇卫生院医疗服务满意度的评价研究OA
Patient satisfaction with healthcare services in rural township health centers of Yimeng Revolutionary Base Area:An evaluation based on SERVQUAL-IPA model
目的 探讨沂蒙革命老区就医患者对乡镇卫生院医疗服务满意度的评价,为提升基层医疗服务质量提供策略.方法 基于SERVQUAL模型设计量表,采用方便抽样法从沂蒙革命老区204家乡镇卫生院中抽取27家,对869名患者进行问卷调查,运用修正IPA分析法对5个维度的服务指标进行优先级排序.结果 患者对乡镇卫生院的总体满意度较高,移情性维度评价均值最高为3.96,有形性维度评价均值最低为3.87.修正IPA分析结果显示:Q3"健康知识宣教"、Q8"在现有的医疗技术水平下,患者能得到满意的诊疗效果"、Q19"您在诊疗后,医护人员会对您进行回访/随访"、Q20"允许您选择为您提供医疗服务的医生、护士或其他医务人员"、Q26"乡镇卫生院重视您的投诉或意见,能够积极解决问题并快速给予回应"等5个指标需要重点改进.结论 沂蒙革命老区乡镇卫生院服务质量与居民的期望存在差距,政府需关注乡镇卫生院服务全覆盖,提高医疗资源分配的效率与公平,完善随访制度,促进优质医疗资源下沉.
Objective To investigate the satisfaction of patients seeking medical care in township health centers in the Yimeng Revolutionary Base Area with medical services and to provide strategies for improving the quality of primary healthcare services.Methods A scale was designed based on the SERVQUAL model.Conven-ience sampling was used to select 27 out of 204 township health centers in the Yimeng Revolutionary Base Area,and 869 patients were surveyed via questionnaires.Modified Importance-Performance Analysis(IPA)was applied to prioritize service indicators across five dimensions.Results Patients reported relatively high overall satisfaction with township health centers.The mean score for the empathy dimension was the highest at 3.96,while the tangi-bility dimension had the lowest mean score of 3.87.The modified IPA results identified five indicators requiring priority improvements:Q3"Health knowledge education,"Q8"Patients can receive satisfactory diagnosis and treatment under the existing medical technology,"Q19"Healthcare providers conduct follow-up visits after diagno-sis and treatment,"Q20"Patients are allowed to choose their healthcare providers(doctors,nurses,or other medical staff),"and Q26"Township health centers value patients'complaints or suggestions,actively solve prob-lems,and provide prompt responses."Conclusions There are gaps between the service quality of township health centers in the Yimeng Revolutionary Base Area and residents'expectations.It is necessary for the governments to focus on ensuring full coverage of health services in township health centers,improving the efficiency and equity of health resource allocation,enhancing follow-up systems,and promoting the downward flow of high-quality health resources to grassroots levels.
