Abstract

Objective:To explore the application value of magnetic resonance imaging in the diagnosis of meniscal degeneration and meniscal tear.Method:The clinical and imaging data of 76 patients with meniscal injury who visited the Department of Imaging,the First People's Hospital of Fuzhou City and 30 healthy people who underwent knee joint examination in the hospital from August 2021 to August 2024 were retrospectively analyzed.Receiver operating characteristic(ROC)curves were constructed to determine the critical value and diagnostic efficacy of magnetic susceptibility in the diagnosis of meniscal tears.Result:Comparability in baseline characteristics including gender distribution,mean age,BMI,and disease duration was observed between the meniscus injury group and the healthy control group(P>0.05).Using arthroscopy as the gold standard(with meniscal tear defined as positive and degeneration as negative),conventional MRI for diagnosing meniscal tears yielded the following results:33 true positives(actual tears correctly diagnosed by MRI),3 false positives(actual degeneration misdiagnosed as tears by MRI),3 false negatives(actual tears missed by MRI and diagnosed as degeneration),and 37 true negatives(actual degeneration correctly diagnosed by MRI).Based on these data,the diagnostic metrics were calculated as follows:accuracy 92.1%(70/76),sensitivity 91.7%(33/36),specificity 92.5%(37/40),missed diagnosis rate 8.3%(3/36),and misdiagnosis rate 7.5%(3/40).Magnetic susceptibility measurements revealed:healthy group median 0.040 ppm(Q1:0.028 ppm,Q3:0.052 ppm),degeneration group median-0.025 ppm(Q1:-0.032 ppm,Q3:-0.003 ppm),and tear group median-0.130 ppm(Q1:-0.235 ppm,Q3:-0.025 ppm),a statistically significant difference was observed among the three groups(P<0.001).The magnetic susceptibility value was significantly higher in the healthy group compared to both the meniscal degeneration group(P<0.001)and the meniscal tear group(P<0.001).Furthermore,the magnetic susceptibility value was significantly lower in the tear group than in the degeneration group(P<0.001).At a cutoff value of-0.062 ppm,the maximum Youden index was 0.87[95%CI(0.860,1.000)].The area under the ROC curve for diagnosing meniscal tears was 0.945,with a sensitivity of 87%and a specificity of 86%.Conclusion:Magnetic sensitivity quantitative technology has superior diagnostic efficacy for meniscus tears,and can accurately distinguish tears from degenerative pathological changes.It is an effective imaging screening scheme for preoperative evaluation of arthroscopy.
