Construction and Practical Effect of Pharmaceutical Care Model Based on Intelligent Pharmaceutical Information System and Therapeutic Drug Monitoring Platform

Abstract
Objective To optimize the workflow of resident pharmacists and improve the level of pharmaceutical care.Methods Resident pharmacists in the hematology department constructed a streamlined and normalized pharmaceutical care model based on the intelligent pharmaceutical information system and therapeutic drug monitoring(TDM)platform,in order to achieve personalized and precise medication.The changes in pharmaceutical care indicators such as order review,problem orders,pharmaceutical care,medication advice,medication education,medication consultation,quantity,personalized medication guidance,and adverse reaction reporting before(from January to December 2023)and after(from January to September 2024)the implementation of the new pharmaceutical care model were compared.Results After the implementation of the new pharmaceutical care model,there were 38 286 medical orders reviewed,103 problematic medical orders,175 cases of pharmaceutical care provided,65 items of medication recommendations,706 items of medication education,69 items of medication consultations,56 cases of personalized medication guidance,and 42 cases of reported adverse reactions,all of which improved compared to those before implementation.Conclusion The pharmaceutical care model based on intelligent pharmaceutical information system and TDM platform can improve the work efficiency of resident pharmacists in the hematology department,improve various pharmaceutical care indicators,and fully play the professional leading role of clinical pharmacists.关键词
