基于"急性虚证"理论探讨老年性重症肺炎的病机与治疗
Exploring the Pathogenesis and Treatment of Senile Severe Pneumonia Based on the Theory of"Acute Deficiency Syndrome"

Abstract
Severe pneumonia is a common critical illness in clinical practice,characterized by complex condi-tions,rapid progression,and numerous complications,with high morbidity and mortality rates among the elderly."Acute deficiency syndrome"is a new concept in traditional Chinese medicine(TCM)describing pathological states,which is gaining increasing recognition in the field of TCM critical care.This paper outlines the theoretical connotation of"Acute deficiency syndrome"and applies this theory to analyze the etiology and pathogenesis of se-nile severe pneumonia:initial stage involves dysfunction of weifen and qifen,allowing external pathogenic toxins to invade internally;middle stage features heat-toxin,phlegm,and blood stasis consuming heart and lung yin and blood;late stage is marked by unstable kidney qi and impending separation of yin and yang.Clinical observation reveals that deficiency syndromes accompany the entire course of severe pneumonia.Based on the fundamental principle that"treating the root cause is also feasible in acute conditions",clinical experience is summarized,and a three-stage treatment strategy is proposed:1)Protecting weifen and consolidating superficies,strengthening the spleen to benefit the lung and expel pathogens in the initial stage;2)Resolving phlegm and removing stasis,nour-ishing the heart and lung to prevent deterioration in the middle stage;3)Promoting mutual generation of metal and water,tonifying and consolidating kidney qi to prevent collapse in the late stage.This provides a novel per-spective for the clinical diagnosis and treatment of senile severe pneumonia.Additionally,a typical clinical case is presented at the end of the paper to provide clinical evidence.关键词
