Abstract

Objective To evaluate the management effectiveness of different blood glucose management models in primary care in China,and to provide a reference for the effective control of patients' blood glucose.Methods With fasting blood glucose(FBG),2 h postprandial blood glucose(2 hPG),and glycated hemoglobin(HbA1C)as evaluation indexes,the Cochrane 5.1.0 bias risk assessment tool was used to evaluate the quality of the included studies.Stata 17.0 and RevMan 5.4 softwares were used to conduct Meta-analysis on the 16 included studies related to different blood glucose management models in primary care.These studies focused on self-management groups for community-based patients,new models of health education,standardized management,multimodal health education,pharmaceutical care provided by clinical pharmacists,medication therapy management in the context of the healthcare consortium model,and primary hospital-community health intervention models.Results Compared with that in the control group,the FBG level in the intervention group significantly decreased[MD=-1.33,95%CI(-1.71,-0.94,P<0.000 1].Subgroup analysis was conducted based on intervention time(3,6,12 months),and the results showed that the FBG levels in the intervention group after 3 months[MD=-2.17,95%CI(-2.61,-1.73),P<0.000 01],6 months[MD=-0.99,95%CI(-1.28,-0.70),P<0.000 01],and 12 months[MD=-1.03,95%CI(-1.56,-0.50),P<0.000 01]were significantly lower than those in the control group.Compared with that in the control group,the 2 hPG levels in the intervention group significantly decreased[MD=-1.68,95%CI(-2.11,-1.25),P<0.000 01].Subgroup analysis was conducted based on intervention time(3,6,12 months),and the results showed that the 2 hPG levels in the intervention group after 3 months[MD=-1.52,95%CI(-2.58,-0.46),P=0.005],6 months[MD=-1.55,95%CI(-1.90,-1.19),P<0.000 01],and 12 months[MD=-1.76,95%CI(-2.61,-0.91),P<0.000 01]were significantly lower than those in the control group.Compared with that in the control group,the HbA1C level in the intervention group significantly decreased[MD=-1.32,95%CI(-1.78,-0.85),P<0.000 01].Subgroup analysis was conducted based on intervention time(3,6,12 months),and the results showed that the HbA1C levels in the intervention group after 3 months[MD=-1.83,95%CI(-2.92,-0.74),P<0.000 01],6 months[MD=-1.01,95%CI(-1.57,-0.44),P<0.000 01],and 12 months[MD=-1.22,95%CI(-1.57,-0.87),P<0.000 01]were significantly higher than those in the control group.Conclusion Compared with those in the general management mode,various blood glucose management models in primary care have shown significant effects in controlling blood glucose indexes such as FBG,2 hPG,and HbA1C.
