Abstract
Objective To explore the establishment of a new model of chronic medication management in the community under a close medical alliance.Methods Based on the SWOT analysis,the strengths,weaknesses,opportunities,and threats of the vertical management in the close medical alliance between the Beijing Tsinghua Changgeng Hospital and Tiantongyuan North Community Health Service Center(referred to as the community)in the management of chronic diseases were analyzed.Scientific medication measures for chronic disease management in the community were put forward according to different element combinations.Results The strengths of implementing the vertical management of the close medical alliance for the chronic disease medication in the community were the unified coordination of medical staff,the increased accessibility of chronic disease drugs,the improvement of chronic disease management ability in community,and the more reasonable allocation of medical resources.The weakness was the decrease in community self-management initiative.The opportunity was that the construction of the close medical alliance had policy and management support.The threat was that the new management model required multi-party cooperation.Conclusion It is suggested that based on the guarantee of homogeneous pharmaceutical care,multi-team cooperation should be relied on to establish a performance appraisal reward mechanism and talent training mechanism that adapt to the community,and share resources,in order to scientificlly manage chronic disease medication in the community.关键词
