补荥泻输针刺法治疗中风后手指屈曲拘挛的临床研究
Clinical Study of the Tonifying Ying and Reducing Shu Acupuncture in the Treatment of Post-Stroke Finger Flexion Contracture

Abstract
Objective:To observe the efficacy of the tonifying Ying and reducing Shu acupuncture in the treatment of post-stroke finger flexion contracture.Methods:110 patients with post-stroke finger flexion contracture were randomly divided into the study group and the conventional group,with 55 patients in each group.The conventional group was treated with conventional rehabilitation training and conventional acupuncture;on which basis,the study group was also treated with the tonifying Ying and reducing Shu acupuncture.Both groups were treated for 2 weeks.TCM symptom scores of both groups were evaluated.Before and after the treatment,assessments were conducted using the Modified Ashworth Spasticity Scale,the FMA-hand,the Neurological Deficit Scale,and the Modified Lindmark Scale to evaluate finger spasticity,hand function,neurological deficits in the hand,and motor impairments.Additionally,measurements were taken of the distance from the tip of the middle finger to the midpoint of the distal palmar crease on the volar wrist and from the tip of the thumb to the tip of the little finger in maximum finger extension.The F-wave latency of the median nerve in the affected limb was also recorded via electromyography.Results:After the treatment,the TCM symptorn scores,Modified Ashworth Spasticity Scale scores,and hand neurological deficit scores were all significantly reduced(P<0.05);meanwhile,the FMA-hand scores and the modified Lindmark scores were significantly increased(P<0.05);the distance from the tip of the middle finger to the midpoint of the distal palmar crease on the volar wrist and the distance between the thumb tip and the little finger tip in maximal finger extension were also significantly increased in the two groups(P<0.05).Compared with the control group,the study group exhibited significantly lower scores in finger flexion spasm,finger joint pain,joint stiffness,local cold intolerance and warmth preference,local coldness,Modified Ashworth scores,and neurological deficit scores(P<0.05).The FMA-hand scores,modified Lindmark scores,the distance from the tip of the middle finger to the wrist crease midpoint,the thumb-to-little finger distance in maximal extension,and the F-wave latency of the median nerve in the affected limb were all significantly higher in the study group than those in the control group(P<0.05).Conclusion:The tonifying Ying and reducing Shu acupuncture can effectively improve clinical symptoms in patients with post-stroke finger flexion spasm,alleviate motor dysfunction of the hand,and enhance motor flexibility and muscle tone.Its mechanism may be related to enhanced conduction of median nerve electromyographic signals,making it suitable for clinical promotion and application.关键词
