从"筋骨平衡"探析青少年特发性脊柱侧弯的诊疗思路
To Explore the Diagnosis and Treatment of Adolescent Idiopathic Scoliosis from the Perspective of"Balance of Muscles and Bones"

Abstract
Adolescent idiopathic scoliosis(AIS)is a common spinal deformity that,in Traditional Chinese Medicine(TCM),is classified under the pattern known as"Gui bei".Although both the prevalence and incidence of AIS con-tinue to rise worldwide,its precise etiology and pathogenesis remain unclear.Drawing upon recent international and domestic advances in the pathogenesis of spinal deformities and in TCM sinew-bone theory,we adopt the"sinew-bone balance"paradigm as our conceptual foundation.We examine how the sinew-bone balance of the spine sustains coor-dinated motor function and systematically expound the TCM physiological concept that"the sinews bind the bones to facilitate joint movement"and the pathological reciprocity captured by the dictum"when bones are aligned,sinews be-come supple;when sinews are supple,bones remain aligned".By integrating current evidence,we investigate dynamic sinew-bone imbalance as the core problem in AIS and delineate the pathological cascade through which this imbalance disrupts spinal alignment.Grounded in the clinicopathological features of AIS and guided by the holistic principles of TCM,our diagnostic and therapeutic framework-summarized as"integrating static and dynamic assessment and giving equal weight to bones and sinews"—offers an innovative theoretical foundation for clarifying the etiology and patho-genesis of AIS and for optimizing clinical diagnosis and treatment.关键词
