Abstract

Objective:To observe and compare the therapeutic efficacy of transcranial direct current stimulation,passive exercise and their combination in the treatment of knee osteoarthritis,and to explore their possible mechanisms.Methods:Fifty eight-week-old fe-male SD rats,with a body weight of 200±20 g,were randomly divided into the blank control group,the model control group,the transcranial direct current stimulation(tDCS)group,the passive exercise group,and the combined tDCS and passive exercise treat-ment group(combined treatment group)according to the random block method,each group consisted of 10 rats.Except for the blank control group,the other groups established the KOA model by injecting sodium iodacetate into the right knee joint of the rats.The model control group was not intervened.The tDCS group received 20-minute tDCS treatment every day for 8 days.The passive exer-cise group performed knee joint passive exercise training twice a day with 300 cycles each time for 8 consecutive days.The com-bined treatment group received both tDCS and passive exercise treatment simultaneously.Pain tests were conducted using a biped balance pain meter before and on days 1,4,7,and 14 after modeling,as well as on day 8 after treatment.The concentrations of inter-leukin 10(IL-10),interleukin 6(IL-6),and tumor necrosis factor-α(TNF-α)in the serum and joint fluid of each group were mea-sured by ELISA.The femoral end cartilage of the right knee joint of each group was sectioned for hematoxylin-eosin staining(HE),modified safranin O-fast green staining,and toluidine blue staining.The degree of articular cartilage injury was evaluated using the Mankin score.The L3~L5 dorsal root ganglia(DRG)of the rats were taken,and the expression of nerve growth factor(NGF)and cal-citonin gene-related peptide(CGRP)in the DRG was detected by immunohistochemistry.Results:(1)The results of pain measure-ment in rats showed that,after modeling,except for the blank control group,the proportion of weight-bearing on the right knee of rats in the other groups decreased.After 8 days of treatment,the proportion of weight-bearing capacity of the right knee in each treat-ment group increased,and the increase was most significant in the combined treatment group.(2)The pathological observation of rat cartilage showed that in the model group,the arrangement of chondrocytes was disordered,the number was reduced,the matrix was decreased and the staining was uneven,the cartilage layer was thin,and the tide line was blurred.In the treatment group,the arrange-ment of chondrocytes was regular,the number increased,the staining of the matrix was uniform,the cartilage layer was thickened,and the tide line was clear.The effect of the combined treatment group was more significant.(3)The Mankin score of rat cartilage showed that the blank control group had the lowest score(P<0.001),and the model control group had the highest score(P<0.001).The score of the combined treatment group was lower than that of the transcranial direct current group and the passive movement group(P<0.001),while there was no statistically significant difference between the transcranial direct current group and the passive movement group(P>0.05).(4)The detection of IL-10 concentration in rat serum showed that the concentration in the blank control group was the highest(P<0.001),and that in the model control group was the lowest(P<0.001).The concentration in the combined treatment group was higher than that of the transcranial direct current group and the passive exercise group(P<0.001),while there was no statistically significant difference between the transcranial direct current group and the passive movement group(P>0.05).(5)The concentration of IL-6 and TNF-α in the joint fluid of normal rats was the lowest(P<0.001),and that of the model group was the highest(P<0.001).The concentration in the combined direct current group was lower than that of the transcranial direct current group and the passive movement group(P<0.001),while there was no statistically significant difference between the transcranial di-rect current group and the passive movement group(P>0.05).(6)The detection of the ratio of NGF and CGRP positive area in rat DRG showed that the blank control group was lower than other groups(P<0.001),and the model control group was higher than other groups(P<0.001).The combined treatment group was lower than the transcranial direct current group and the passive exercise group(P<0.001),while there was no statistically significant difference between the transcranial direct current group and the passive exercise group(P>0.05).Conclusion:Transcranial direct current combined with passive exercise can reduce the pain of osteoarthritis model rats.The mechanism may be:first,it can reduce the inflammatory pain by reducing the content of inflammatory factors IL-6 and TNF-α in synovial fluid,and second,it can reduce the pain sensitization by reducing the expression of pain factors NGF and CGRP in DRG.
