Abstract
Objective:To compare the effects of totally laparoscopic right hemicolectomy and laparoscopic-assisted right hemi-colectomy on postoperative intestinal recovery.Methods:A retrospective analysis was conducted on clinical data from 108 patients who underwent laparoscopic right hemicolectomy between Aug.2021 and Jan.2023.The patients were divided into total group(n=55)and assisted group(n=53)based on the surgical approach.Surgical indicators,inflammatory markers,and postoperative recovery outcomes were compared between the two groups.Results:There were no significant differences in baseline characteristics,number of lymph nodes dissected,or resected specimen length between the two groups.The total group had a significantly longer operative time than the assisted group[(158.15±15.65)min vs.(149.62±14.98)min,P=0.005].However,the total group showed significantly shorter times to:first postoperative flatus[(2.39±0.43)d vs.(2.67±0.59)d,P=0.006],ambulation[(1.88±0.38)d vs.(2.25±0.52)d,P<0.001],and hospital stay[(6.53±1.03)d vs.(7.02±1.34)d,P=0.036].The incidence of postoperative ileus was significantly low-er in the total group(1 case vs.6 cases,P=0.045).Preoperative levels of tumor necrosis factor-α,interleukin-6,C reactive protein,cortisol,adrenocorticotropic hormone,and epinephrine showed no significant differences between the two groups(P>0.05).On post-operative day 3,all above indicators increased in both groups,the total group had significantly lower levels of these markers(P<0.05).Conclusions:Totally laparoscopic right hemicolectomy can shorten the time to first flatus,ambulation,and hospital stay,and reduce the incidence of postoperative ileus,showing more advantages in postoperative intestinal function recovery.关键词
