腹腔镜系膜剥离法阑尾切除术联合加速康复外科理念治疗急性穿孔性阑尾炎的临床观察
Clinical observation of the laparoscopic mesoappendiceal dissection method combined with the enhanced recovery after surgery concept in the treatment of acute perforated appendicitis

Abstract
Objective:To explore the clinical efficacy of laparoscopic mesoappendiceal dissection combined with the enhanced recovery after surgery(ERAS)concept in the treatment of acute perforated appendicitis.Methods:A total of 107 patients with acute perforated appendicitis admitted from Aug.2023 to Jun.2024 were selected and randomly assigned to two groups.The conventional group(n=53)underwent laparoscopic mesoappendiceal dissection combined with conventional interventions,while the treatment group(n=54)was treated with laparoscopic mesoappendiceal dissection combined with the ERAS concept.The surgical indicators,pain le-vels,inflammatory markers,intestinal barrier function,quality of life,and overall complication rates were compared between the two groups.Results:The time to first flatus,ambulation time,and hospital stay in the treatment group were significantly shorter than those in the conventional group(P＜0.05).The pain visual analogue scale score,C reactive protein,interleukin-6,tumor necrosis factor-α,diamine oxidase,and D-lactate levels in the treatment group were significantly lower than those in the conventional group(P＜0.05).The quality of life score was significantly higher in the treatment group(P＜0.05).There was no significant difference in the overall complication rate between the two groups(P＞0.05).Conclusions:For acute perforated appendicitis,laparoscopic mesoappendiceal dis-section combined with the ERAS concept is superior to laparoscopic mesoappendiceal dissection combined with conventional interven-tions,offering advantages in shortening recovery time,alleviating postoperative pain,and improving quality of life.关键词
