经脐单孔与三孔法腹腔镜全腹膜外腹股沟疝修补术的对比研究
Comparison between transumbilical single-port and conventional three-port techniques in laparoscopic totally extraperitoneal inguinal hernia repair

Abstract
Objective:To evaluate the safety and efficacy of transumbilical single-port laparoscopic totally extraperitoneal(TEP)inguinal hernia repair in adults and summarize clinical experience.Methods:The clinical data of 225 patients with inguinal her-nia treated between Oct.2022 and May 2024 were retrospectively analyzed.Among them,90 patients underwent transumbilical single-port TEP(single-port group),while 135 patients underwent conventional three-port TEP(three-port group)as controls.The two groups were compared in terms of hernia type,operative time,intraoperative blood loss,postoperative 24-hour visual analog scale pain scores,postoperative complications,postoperative hospital stay,total hospital stay,hospitalization costs,patient satisfaction regarding abdominal wall at 1 month postoperatively,and hernia recurrence at 6-month follow-up.Results:There were no significant differences in baseline clinical characteristics between the two groups(P＞0.05).The single-port group had higher hospitalization costs than the three-port group[7700(7300,9900)yuan vs.7400(6900,9500)yuan,P=0.016].The transumbilical single-port group demonstrated signifi-cantly lower postoperative 24-hour pain scores[2(1,3)vs.3(2,4),P＜0.001]and higher abdominal wall satisfaction at 1 month postoperatively(P=0.003).There were no statistically significant differences between the two groups in operative time[60(40,70)min vs.60(46,76)min,P=0.421],intraoperative blood loss[5(5,5)mL vs.5(5,5)mL,P=0.831],total hospital stay[4(3,6)d vs.4(4,5)d,P=0.408],postoperative hospital stay[1(1,2)d vs.2(1,2)d,P=0.064],or postoperative complications(P=0.159).No cases of hernia recurrence were observed during the 6-month follow-up in either group.Conclusions:Transumbilical single-port TEP is a safe and effective approach for adult inguinal hernia repair,offering superior cosmetic outcomes.It is worthy of wider clinical applica-tion.关键词
