Therapeutic effect of three-port LC with abdominal wall puncture retractor on acute calculous cholecystitis and its impact on serum PA,CRP and IL-8

Abstract
Objective To explore the therapeutic effect of three-port laparoscopic cholecystectomy(LC)with abdominal wall puncture retractor on patients with acute calculative cholecystitis and its impact on serum prealbumin(PA),C reactive protein(CRP),and interleukin-8(IL-8).Methods A total of 146 patients with acute calculous cholecystitis admitted from June 2022 to May 2024 were selected and divided into the three-port method group(n=75)and the traditional group(n=71)according to different surgical methods.The three-port method group was treated with three-port LC with abdominal wall puncture retractor,while the traditional group was treated with traditional four-port LC.The surgical indicators,length of hospital stay,the Visual Analogue Scale(VAS)pain scores before the operation and at 12 and 24 h after the operation,the levels of serum PA,lipase(LPS)and inflammatory factors[CRP,IL-8,tumor necrosis factor-α(TNF-α)]of the patients before the operation and at 3 d after the operation,as well as the incidence of postoperative complications were compared between the two groups.Results The duration of operation,intraoperative blood loss and length of hospital stay in the three-port method group were all less or shorter than those in the traditional group(P<0.01).At 12 and 24 h after the operation,the VAS pain scores in the three-port method group were lower than those in the traditional group(P<0.01).At 3 d after the operation,the serum PA levels in both groups increased,while the serum LPS levels decreased.Moreover,the serum PA level in the three-port method group was higher than that in the traditional group,while the serum LPS level was lower than that in the traditional group(P<0.05,P<0.01).At 3 d after the operation,the levels of serum CRP,IL-8 and TNF-α in both groups decreased,which were lower in the three-port method group than in the traditional group(P<0.05,P<0.01).The total incidence of postoperative complications in the three-port method group was lower than that in the traditional group(P<0.05).Conclusion The three-port LC with abdominal wall puncture retractor can effectively promote the postoperative recovery of patients with acute calculous cholecystitis,reduce postoperative pain,improve inflammatory response,and has a relatively low incidence of postoperative complications.关键词
