医疗安全不良事件上报现状分析与改进策略研究
Analysis of the current status of medical safety adverse event reporting and research on improvement strategies

Abstract
Objective To systematically analyze the reporting characteristics of medical quality and safety adverse events in a tertiary first-class specialized neurosurgery hospital in Tianjin in 2024,identify risk points unique to specialized hospitals,and propose targeted improvement strategies.Methods A retrospective analysis was conducted based on 861 valid adverse event reports from the hospital's reporting system.Multi-dimensional analysis was performed according to national classification stand-ards(event type,severity level,location of occurrence).Innovatively,custom classifications(such as"Absence of Family Care-giver","Poor Patient Compliance",etc.)were applied to deeply explore risk factors specific to specialized neurosurgery hospi-tals.Results Clinical departments accounted for 90.10％of reports.Medical management-related events(28.5％)and securi-ty management-related events(22.1％)were the most prominent types.Level Ⅲ and Ⅳ events together constituted 95.91％of the total,indicating timely intervention for most risks.Key findings included:"Absence of Family Caregiver"was the single most frequent event category(29.0％),Doctor-patient communication issues(8.4％)and poor patient compliance(8.2％)formed a significant communication-related risk cluster,among hospital safety hazards,microwave oven usage risks accounted for 31.03％.Conclusion A comprehensive adverse event reporting system with multi-departmental collaboration should be established.Le-veraging information technology for real-time monitoring and early warning is crucial.Standardized reporting procedures must be implemented.Enhancing the identification and management of high-risk groups,implementing predictive risk nursing,strengthe-ning medical safety education for patients and families,optimizing doctor-patient communication,and reinforcing medical human-istic care are essential.These multi-faceted approaches provide empirical evidence and a practical framework for specialized hos-pitals to implement national medical quality improvement action plans.关键词
