"辛开苦降"法及食疗中药治疗干眼思路探析
Exploration of the"acrid medicines open and bitter medicines descend"method and dietary Chinese medicines therapy in treating dry eye syndrome

Abstract
Dry eye syndrome(DES)is one of the most common chronic and refractory ocular surface disorders in clinical practice.It has complex etiologies,a prolonged disease course,and often presents with patterns involving both deficiency and excess,as well as cold-heat complexity.The therapeutic principle of"acrid medicines open and bitter medicines descend",proposed by ZHANG Zhongjing for the treatment of patterns with cold and heat in complexity,employs acrid-flavored medicines to activate,moisten,and disperse,and bitter-flavored medicines to drain,dry,and fortify.When combined,they harmonize cold and heat,regulate the ascending and descending of qi movement,and balance yin and yang.Patients with refractory DES may also present with pathogenesis of cold and heat in complexity,such as spleen cold with stomach heat pattern due to impaired ascending-descending function of spleen and stomach qi movement,and upper heat and lower cold pattern resulting from kidney yin-yang imbalance.In the clinical treatment and dietary therapy of such complex cases of DES,attention should be paid to the application of the"acrid medicines open and bitter medicines descend"method to reconcile cold and heat and restore ocular surface homeostasis.关键词
