Abstract
Objective To investigate the impact of nutrition therapy guided by energy metabolism monitoring on nutritional status and prognosis in patients with severe traumatic brain injury(sTBI).Methods Ninety-two sTBI patients treated at The First Affiliated Hospital of Henan University of Science and Technology from January 2021 to December 2024 were included.Using the start of energy metabolism-guided nutrition as the time node,45 patients managed with routine nutrition from January 2021 to December 2022 comprised the routine group,and 47 patients receiving nutrition therapy under energy metabolism monitoring from January 2023 to December 2024 comprised the nutrition therapy group.Indices were evaluated before and after treatment:nutritional support outcomes[hemoglobin(Hb),total protein(TP),serum albumin(Alb),prealbumin(PA)],inflammatory markers[neutrophils(NEU),procalcitonin(PCT),C-reactive protein(CRP)],gastrointestinal complications,and prognosis[intensive care unit(ICU)length of stay,Glasgow Outcome Scale(GOS)at 28 d after admission,28 dmortality].Results After treatment,Hb,TP,Alb,and PA increased in both groups,with higher levels in the nutrition therapy group than in the routine group(P<0.05).NEU,PCT,and CRP decreased in both groups,with lower levels in the nutrition therapy group(P<0.05).The incidence of gastrointestinal complications was lower in the nutrition therapy group than in the routine group(P<0.05).ICU length of stay was shorter and GOS at 28 d was higher in the nutrition therapy group than in the routine group(P<0.05).The 28 d mortality did not differ between groups(P>0.05).Conclusion Nutrition therapy guided by energy metabolism monitoring significantly improves nutritional status,attenuates inflammatory responses,reduces gastrointestinal complications,and facilitates early recovery in patients with sTBI.关键词
