胰腺癌根治术术前HELPP评分及多期增强CT的临床意义研究

Abstract
Objective To explore the clinical significance of the preoperative Heidelberg Prognosis Pancreatic Cancer(HELPP)score and multi-phase enhanced CT in radical surgery for pancreatic cancer.Methods A retrospective study included 70 patients with pancreatic cancer treated at Changge People's Hospital from August 2019 to August 2021.According to 1-year postoperative survival,patients were classified into a death group(n=49)and a survival group(n=21).Preoperative clinical data,HELPP scores,and preoperative pancreatic multiphase CT values were compared between the two groups.Results The proportion of patients older than 65 years was higher in the death group than in the survival group(P＜0.05).The proportion of patients with clinical stage III-IV was higher in the death group than in the survival group(P＜0.05).The proportion of patients with HELPP score＞3 was higher in the death group than in the survival group(P＜0.05).Preoperative pancreatic parenchymal-phase CT values and portal venous-phase CT values were lower in the death group than in the survival group(P＜0.05).Logistic regression analysis showed that age,clinical stage,HELPP score,and preoperative pancreatic vascular-phase CT value were influencing factors for the prognosis of radical surgery for pancreatic cancer(P＜0.05).The area under the ROC curve for predicting the prognosis of radical surgery for pancreatic cancer was 0.845(95%CI:0.748～0.943)(P＜0.05),with sensitivity and specificity of 79.60%and 81.00%,respectively.Conclusion The combination of the preoperative HELPP score and pancreatic multiphase contrast-enhanced CT has certain application value in assessing the prognosis of radical surgery for pancreatic cancer and merits further study.关键词
