Abstract
Objective To observe the clinical efficacy of noninvasive positive pressure mechanical ventilation combined with high-flow oxygen therapy in the treatment of severe pneumonia complicated by respiratory failure.Methods A total of 102 patients with severe pneumonia complicated by respiratory failure admitted to The First Affiliated Hospital of Nanyang Medical College from December 2022 to December 2024 were selected and randomly assigned by a random number table into two groups(51 cases per group).The control group received noninvasive positive pressure mechanical ventilation,and the study group received high-flow oxygen therapy in addition to the treatment given to the control group.Treatment conditions,oxygenation function,and levels of inflammatory factors were compared between the two groups.Results The duration of noninvasive ventilation,total oxygen inhalation time,and length of hospital stay were shorter in the study group than in the control group,and the reintubation rate within 28 d,mortality within 28 d,and incidence of ventilator-associated pneumonia were lower than those in the control group,with statistically significant differences(P<0.05).After treatment,arterial partial pressure of oxygen(PaO2),oxygenation index(OI),and oxygen saturation(SpO2)increased in both groups,and were higher in the study group than in the control group(P<0.05).After treatment,interleukin(IL)-6,IL-8,and IL-33 levels decreased in both groups,and were lower in the study group than in the control group(P<0.05).Conclusion Noninvasive positive pressure mechanical ventilation combined with high-flow oxygen therapy for severe pneumonia complicated by respiratory failure can shorten the duration of respiratory support,reduce the reintubation rate,mortality,and complication rate within 28 d,improve oxygenation function,and attenuate the inflammatory response.关键词
