微镜下多点架桥技术在椎基底动脉延长扩张综合征所致三叉神经痛微血管减压术中的应用
Microscope-guided Multipoint Bridging Technique in Microvascular Decompression for Trigeminal Neuralgia Due to Vertebrobasilar Dolichoectasia

Abstract
Objective To explore the application and effectiveness of the microscope-guided multipoint bridging technique in microvascular decompression for trigeminal neuralgia due to vertebrobasilar dolichoectasia.Methods Twenty-one patients with trigeminal neuralgia caused by vertebrobasilar dolichoectasia admitted in Taihe Hospital,Hubei University of Medicine from January 2019 to July 2024 were retrospectively analyzed.Among them,18 patients underwent microvascular decom-pression using the microscope-guided multipoint bridging technique for the transposition of responsible vessels;3 patients received microscope-guided local decompression with padding.Intraoperative techniques,postoperative efficacy,recur-rence,and complications were observed.Results All microvascular decompression surgeries using the microscope-guided multipoint bridging technique were successfully completed,achieving a 100%success rate.Postoperative pain relief was at-tained in 100%of cases.Follow-ups of 12 to 72 months revealed no recurrence in any patient,with a low incidence of com-plications.The complications included 1 case of hearing loss,1 case of masticatory muscle weakness with facial numbness,and 1 case of facial paralysis that had resolved.No severe complications,such as brainstem ischemia,occurred.The local decompression with padding also achieved a 100%success rate,with 1 case of delayed healing and recurrence approximate-ly 1 year after surgery.Conclusion The microscope-guided multipoint bridging technique in microvascular decompression for trigeminal neuralgia due to vertebrobasilar dolichoectasia effectively suspends the responsible vessels and alleviates com-pression on the nerve,with clear operative vision with safe and controlled procedures.This technique helps achieve com-plete decompression of the trigeminal nerve,contributing to clinical cure and prevention of postoperative recurrence.It is a safe,effective,and practical surgical method.关键词
