血清维生素D缺乏的不明原因复发性流产患者CD19+细胞水平与不良妊娠结局的关系
Association Between CD19+Cells and Adverse Pregnancy Outcomes in Women with Unexplained Recurrent Spon-taneous Abortion and Vitamin D Deficiency

Abstract
Objective To explore the relationship between peripheral blood CD19+cells and adverse pregnancy outcomes in women with unexplained recurrent spontaneous abortion(RSA)and vitamin D deficiency,and to evaluate the predictive value of CD19+cells.Methods A retrospective analysis was conducted on the clinical data of 116 RSA patients(≥2 mis-carriages)who visited the Reproductive Medicine Center of Xiangyang No.1 People's Hospital from October 2021 to Octo-ber 2023.The patients were divided into an observation group(n=73)and a control group(n=43)based on the the ser-um 25-(OH)D level.Immunological and thrombotic markers were compared between the groups,along with pregnancy out-comes.The patients were further stratified based on subsequent pregnancy outcomes into a delivery group(n=64)and a miscarriage group(n=21).CD19+cell,CD3-CD56+cells,complement C3,and complement C4 were compared and ana-lyzed.Logistic regression was performed to identify the risk factors for vitamin D deficiency and RSA,and the receiver oper-ating characteristic(ROC)curves were used to assess the predictive value of CD19+cells for adverse pregnancy outcomes.Results Compared with the control group,the observation group exhibited elevated CD19+and CD3-CD56+,reduced com-plement C3 and C4,and a higher incidence of subsequent miscarriage(P<0.05).The miscarriage group had higher CD19+level than the delivery group(P<0.05).Multivariate logistic regression revealed that high expression of CD19+cells was an independent risk factor for vitamin D deficiency in RSA patients[OR(95%CI):1.143(1.025-1.274)]and an inde-pendent predictor of RSA[OR(95%CI):1.161(1.020-1.322)].The ROC curve analysis demonstrated that CD19+cells had an AUC of 0.730 for predicting adverse pregnancy outcomes.Conclusion Low vitamin D may impair immune tol-erance by regulating CD19+cell proliferation,which contributes to adverse pregnancy outcomes in RSA patients.Therefore,it is feasible to monitor and intervene in vitamin D and CD19+levels for improving pregnancy outcomes.关键词
