神经康复机械手联合经耳迷走神经刺激对脑卒中偏瘫患者上肢运动功能的影响
Effect of Neurorehabilitation Manipulator Combined with Transauricular Auricular Vagus Nerve Stimulation on Upper Limb Motor Function in Stroke Patients with Hemiplegia

Abstract
Objective To investigate the effect of intensive training with neurorehabilitation manipulator and transauricular auricular vagus nerve stimulation(taVNS)on the recovery of upper limb motor function in stroke patients with hemiplegia.Methods A total of 96 stroke patients hospitalized in the Department of Rehabilitation Medicine,Xiangyang No.1 People's Hospital,Hubei University of Medicine from December 2021 to December 2022 were randomly divided into three groups:taVNS group,rehabilitation manipulator group,and combined group,with 32 cases in each group.All three groups re-ceived routine rehabilitation training.Additionally,the taVNS group was treated by taVNS;the rehabilitation manipulator group was treated with neurorehabilitation manipulator;the combined group was given both taVNS and neurorehabilitation manipulator,for 6 weeks in total.Before and after treatment,the cortical latency in motor evoked potential(MEP)and central motor conduction time(CMCT)were measured and analyzed using an electromyography evoked potentiometer.The Fugl-Meyer Assessment for Upper Extremity(FMA-UE),Activities of Daily Living(ADL),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),and National Institutes of Health Stroke Scale(NIHSS)were employed for assessments,and the adverse reactions during treatment were also recorded.Results After taVNS and neurorehabilitation manipulator treatments,FMA-UE and ADL scores in all three groups were higher than those before treatment,and the cor-tical latency in MEP,CMCT,SAS,SDS,and NIHSS scores were reduced(P<0.05),with the combined group exhibiting the most significant improvement(P<0.05).There was no difference in the incidence of adverse reactions among the three groups(P>0.05).Conclusion Both taVNS and neurorehabilitation manipulator can improve the muscle strength of the af-fected upper limb,hand flexibility,and upper limb motor function in stroke patients with hemiplegia.Furthermore,the combined application of both methods yields better results.关键词
