直肠癌新辅助放化疗后病理组织肿瘤退缩分级与长期生存预后的相关性分析

Abstract
Objective To explore the correlation between pathological tumor regression grading(TRG)after neoadjuvant chemoradiotherapy(nCRT)for rectal cancer and the long-term survival prognosis of patients.Methods The clinical data of 70 patients with advanced rectal cancer admitted to Zhoukou First People's Hospital from January 2022 to December 2023 were retrospectively analyzed.The TRG scoring sys-tem of American Association of Pathologists was used to evaluate the degree of tumor regression.The correla-tion between TRG and overall survival rate(OS)and disease-free survival rate(DFS)was analyzed,and uni-variate and multivariate analyses were carried out in combination with clinicopathological features.Results Among the 70 patients,there were 16 cases(22.9％)with TRG grade 0,23 cases(32.9％)with TRG grade 1,21 cases(30.0％)with TRG grade 2,and 10 cases(14.2％)with TRG grade 3.The median follow-up time was 28.5 months.The 3-year OS and DFS of patients with TRG grade 0-1 were significantly higher than those of patients with TRG grade 2-3(92.3％VS 69.4％,85.7％VS 58.1％),and the difference was statis-tically significant(P<0.05).Multivariate analysis showed that TRG grading and TNM staging were indepen-dent prognostic factors affecting the long-term survival of patients.Conclusion The pathological TRG grad-ing after nCRT for rectal cancer is closely related to the long-term survival prognosis of patients.TRG can be used as an important indicator to evaluate the treatment response and predict the prognosis,which is helpful for guiding the formulation of individualized treatment strategies.关键词
