Abstract
Objective To compare the effects of Twin-block and muscle activator in correcting Class Ⅱ mandibular retraction and their impact on the indicators of dental and maxillofacial hard tissue change and soft tissue facial profile status.Methods The data of 72 patients with Class Ⅱ mandibular retraction in General Hospital of Puyang Oilfield,Henan Province(January 2021 to July 2024)were retrospectively analyzed,they were categorized into two groups based on different orthodontic treatment plans,36 patients who underwent Twin-block orthodontics were classified as Group A,while the 36 patients who underwent myofunctional activator orthodontics were classified as Group B.The orthodontic effects of the two groups were compared,as well as the dental and maxillofacial hard tissue change indicators the mandibular body angle(SNB),the mandibular angle(SNA),the mandibular ramus(Go-co),the distance between the aesthetic plane and the lower lip protrusion point(LL-Eline),and the distance between the aesthetic plane and the upper lip protrusion point(UL-Eline).Soft tissue facial profile status indicators(chin soft tissue thickness,chin groove inclination,coverage,and nasolabial angle).Hyoid position distance from hyoid point to pre-pyramidal plane(H-CVP),distance from hyoid point to posterior nasal spine point(H-PNS),distance from hyoid point to palatal plane(H-PP),sagittal diameter of upper airway sagittal diameter of upper airway at the apex of soft palate(AP-sp),sagittal diameter of upper airway at the apex of epiglottis(AP-te),sagittal diameter of upper airway at the hard palate(AP-hp)before and after 6 months of orthodontic treatment.Results There was no statistically significant difference in the excellent and good rates of correction between the two groups(P>0.05).After 6 months of correction,SNB,Go-co and UL-Eline in group A were higher than those in group B,while SNA and LL-Eline were lower than those in group B.The differences were statistically significant(P<0.05).After 6 months of correction,the thickness of the chin soft tissue,the inclination Angle of the chin groove and the nasolabial Angle in group A were higher than those in group B,and the coverage was lower than that in group B.The differences were statistically significant(P<0.05).After 6 months of correction,the H-CVP,H-PNS and H-PP in group A were higher than those in group B,and the difference was statistically significant(P<0.05).After 6 months of correction,the AP-sp,AP-te and AP-hp in group A were higher than those in group B,and the difference was statistically significant(P<0.05).Conclusions Twin-block and muscle activator orthodontic treatments have comparable effects on patients with Class Ⅱ mandibular retraction.However,Twin-block orthodontic treatment is more effective in improving the dental and maxillofacial hard tissue and soft tissue facial profile status,as well as the airway morphology,and correcting the position of the hyoid bone.关键词
