Evaluation of the Effect of Pharmacy Intelligence Platform on the Whole-Process Pharmaceutical Care for Inpatients

Abstract
Objective To standardize the whole-process pharmaceutical care management for inpatients and improve the quality of clinical pharmaceutical services.Methods A pharmacy intelligence platform(PIP)was introduced into a hospital in 2022,and mobile ward rounds were used to conduct pharmaceutical care for inpatients.Clinical pharmacists continuously monitored the key cases prompted by PIP and carried out individualized medication management.For chemotherapy patients,standardized whole-process pharmaceutical care was provided,including admission pharmaceutical assessment,medication reconciliation,medical order review,medication monitoring,medication advice,medication guidance,and medication consultation.Results After the launch of PIP,the medical order review rate for inpatients in the hospital reached 100%.Compared with one year before the launch of PIP,the whole-process pharmaceutical care rate for inpatients increased from 6.91%to 26.58%,the medication reconciliation rate for inpatients increased from 5.92%to 38.70%,the medication advice rate for inpatients increased from 7.32%to 54.32%,the consultation rate for inpatients increased from 6.75%to 25.60%,and the satisfaction rate for inpatients increased from 88.54%to 97.54%.Conclusion The whole-process pharmaceutical care management for inpatients can makes up the shortcomings of traditional ward rounds and pharmaceutical care services,which is of great significance for standardizing clinical pharmaceutical care,transforming the pharmaceutical service model,improving the level of clinical pharmaceutical services,and ensuring the safety of patients'medication.关键词
