从"正邪交争"理论探讨化脓性骨髓炎病因病机及分期辨治思路
Exploring pyogenic osteomyelitis from the perspective of"the struggle between vital qi and pathogenic qi"theory etiology,pathogenesis,and treatment approach based on disease stages

Abstract
The theory of the struggle between righteousness and evil is one of the important theories in traditional Chinese medicine(TCM)for understanding diseases and guiding clinical treatment.The interplay between the two sides-righteousness and evil-determines the changes,development,and outcomes of diseases.Suppurative osteomyelitis(SO)can be classified into acute and chronic forms based on the duration of illness and onset speed.The external causes primarily include invasion by six excesses(external pathogens)and toxic evils,while internal factors mainly involve deficiency of righteous qi.Contributing factors to its onset include excessive sexual activity,irregular diet,and emotional disturbances.In cases where righteous qi is deficient,pathogenic toxins invade through vulnerabilities,leading to obstruction in meridians and stagnation of qi and blood;This represents the primary pathogenesis during the initial stage of disease.Treatment should focus on invigorating blood circulation and promoting qi flow as a priority while preventing further complications.As pathogenic toxins accumulate excessively,they generate heat that leads to pus formation,causing erosion of muscles and bones;thus,treatment at this stage emphasizes expelling evil forces.In the late stages following ulceration when there is a combination of residual evil with deficiency in righteousness,it becomes crucial to support righteousness while simultaneously clearing remaining evils to prevent recurrence after recovery.关键词
