支气管肺泡灌洗液病原靶向二代测序在社区获得性肺炎中的应用
Application of targeted next-generation sequencing of bronchoalveolar lavage fluid in community-acquired pneumonia
摘要
Abstract
Objective To explore the value of targeted next-generation sequencing(tNGS)of bronchoalveolar lavage fluid(BALF)in the diagnosis and treatment of community-acquired pneumonia(CAP).Methods A total of 215 patients diagnosed with CAP in the Yunnan Honghe Prefecture Central Hospital,from January 2023 to January 2024 were collected.All patients were fully prepared within 72 hours of admission,and BALF tNGS testing,microbial culture,sputum culture,blood culture,and other laboratory examinations were performed simultaneously.The patient's admission time,clinical symptoms,signs,chest CT and other data were collected,and the relevant results were retrospectively analyzed.Results Among the detection results of 215 CAP patients,the median age of the group with underlying pulmonary diseases(n=34)was(69.0[58.5,77.5])years,which was greater than that of the group without underlying pulmonary diseases(n=181)(64.0[51.0,75.0])years,and the difference was statistically significant(P<0.05).The positive detection results of tNGS test were 200 cases,with a positive rate of 93.02%,a sensitivity of 92.45%,a specificity of 6.79%,and a positive predictive value of 24.50%.The positive results of traditional pathogen testing were 53 cases,with a positive rate of 24.65%,and the positive rate of tNGS test was 3.77 times higher than that of traditional pathogen testing.Conclusion The positive result of tNGS in BALF is significantly higher than that of conventional pathogenic testing.It can identify CAP pathogens and drug-resistant bacteria in an early and timely manner,form a synergistic effect with traditional pathogen testing,and provide effective help for clinicians in the diagnosis and treatment of CAP.关键词
