Abstract
Objective To report factors influencing the prognosis in sudden sensorineural hearing loss(SSNHL)to improve treatment.Methods A total of 414 patients with unilateral SSNHL,admitted to The Affiliated People's Hospital of Jiangsu University between January 2019 and December 2022,were included in this retrospective study,and categorized as having good prognosis(n=210)or poor prognosis(n=204)based on treatment outcomes.Hearing data before and after treatment were compared to assess treatment efficacy.Factors impacting treatment efficacy were analyzed by one-way analysis,Spearman's correlation and multivariate logistic regression.The area under the receiver operating characteristic curve was used to predict treatment outcome.Results Age,disease duration,LDL levels and incidence of concomitant tinnitus were higher in the poor prognosis group than in the good prognosis group(P<0.05).With low-frequency,high-frequency and all-frequency hearing loss patterns,treatment efficacy was negatively correlated with the degree of hearing loss(P<0.05).Triglycerides(TG)and LDL levels in the good prognosis group were lower than in the poor prognosis group(P<0.001),and high-density lipoprotein(HDL)levels were higher(P<0.001).Lymphocytes,neutrophilic granulocyte,total cholesterol and apolipoprotein B levels in the good prognosis group showed no significant difference compared to the poor prognosis group(P>0.05).The severity of hearing loss,total deafness,and LDL and TG levels were negatively correlated with prognosis(P<0.01),whereas HDL levels were positively correlated with prognosis(P<0.01).The area under the ROC curve was 0.647(95%CI:0.594,0.699)for age,0.585(95%CI:0.530,0.640)for disease duration and 0.651(95%CI:0.598,0.704)for LDL level.Age>47 years,disease duration>10 days,presence of tinnitus,hearing loss pattern and LDL>2.93 mmol/L were identified as independent risk factors affecting prognosis(P<0.05).Conclision The factors influencing the prognosis of patients with sudden sensorineural hearing loss(SSNHL)include age,presence of tinnitus symptoms,high LDL levels,and the type of hearing curve.Clinically,early intervention and targeted treatment should be provided for high-risk patients to improve the therapeutic effect of SSNHL.关键词
