泛免疫炎症值对肾结石输尿管软镜碎石术后全身炎症反应综合征的预测价值
The Predictive Value of Pan-immune Inflammatory Value for Systemic Inflammatory Response Syndrome after Flexible Ureteroscopic Lithotripsy for Kidney Stones

Abstract
Objective To explore the predictive value of the pan-immune-inflammation value(PIV)for systemic inflammatory response syndrome(SIRS)after flexible ureteroscopy lithotripsy(FURL)in patients with kidney stones.Methods A total of 202 patients with kidney stones who underwent FURL at the Shaanxi Armed Police Corps Hospital from February 2022 to February 2025 were selected as the study subjects.Based on the occurrence of SIRS within two weeks after surgery,the patients were divided into a non-SIRS group(172 cases)and a SIRS group(30 cases).Clinical data were collected and preoperative PIV was calculated for all patients.The predictive value of PIV for SIRS after FURL was analyzed using receiver operating characteristic(ROC)curve analysis,and multivariate logistic stepwise regression was used to explore influencing factors for SIRS after FURL in patients with kidney stones.Results The neutrophil-to-lymphocyte ratio(NLR),monocyte-to-lymphocyte ratio(MLR),platelet-to-lymphocyte ratio(PLR),and PIV in the SIRS group were higher than those in the non-SIRS group(P<0.05).ROC curve analysis showed that the areas under the curve(AUC)(95%CI)of NLR,MLR,PLR,and PIV for predicting SIRS after FURL were 0.791(0.746-0.841),0.734(0.684-0.779),0.815(0.770-0.865),and 0.903(0.858-0.953),respectively.The AUC of PIV was higher than those of NLR,MLR,and PLR(Z=10.280,9.124,and 11.757,respectively;all P<0.05).The proportions of patients with a maximum stone diameter≥2 cm,multiple stones(≥2),preoperative fever,operation time≥60 minutes,and hemoglobin decrease≥10%were higher in the SIRS group than in the non-SIRS group(P<0.05).Multivariate analysis showed that operation time(OR=2.012,95%CI:1.019-3.971),preoperative fever(OR=2.063,95%CI:1.099-3.870),NLR(OR=2.552,95%CI:1.280-5.088),and PIV(OR=2.948,95%CI:1.771-4.907)were independent influencing factors for SIRS after FURL in patients with kidney stones(P<0.05).Conclusion Preoperative PIV can effectively predict the risk of SIRS after FURL in patients with kidney stones and is an independent influencing factor for SIRS after FURL.关键词
