动力加压钢板与锁定加压钢板联合骨移植治疗胫腓骨创伤性骨折不愈合疗效比较
Comparison of Efficacy between Dynamic Compression Plate and Locking Compression Plate Combined with Bone Graft in the Treatment of Traumatic Tibiofibular Fracture Nonunion

Abstract
Objective To analyze and compare the clinical efficacy of dynamic compression plate(DCP)versus locking compression plate(LCP)combined with bone graft in the treatment of traumatic nonunion of tibial and fibular fractures.Methods A total of 153 patients with traumatic nonunion of tibial and fibular fractures admitted to the General Hospital of Northern Theater Command from January 2020 to June 2024 were retrospectively selected and divided into two groups according to the internal fixation method:the DCP group(78 cases)treated with DCP internal fixation plus bone graft,and the LCP group(75 cases)treated with LCP internal fixation plus bone graft.Perioperative indicators,fracture healing,internal fixation stability,complications,and knee function were compared between the two groups.Results The operation time in the DCP group was shorter than that in the LCP group,while the intraoperative blood loss was greater and the incision length was longer in the DCP group(all P<0.05).At 3 months postoperatively,fracture healing was better in the LCP group than in the DCP group(P<0.05).There were no significant differences in the incidence of plate loosening or plate fracture between the two groups at 3 and 6 months after surgery(P>0.05).The total complication rate in the LCP group(6.67%)was lower than that in the DCP group(17.95%)(P<0.05).At 3 months after surgery,the Lysholm knee score(LKS)in the LCP group was higher than that in the DCP group(P<0.05),but there was no significant difference in LKS between the two groups at 6 months after surgery(P>0.05).Conclusion Both DCP and LCP combined with bone graft resulted in similar recovery of knee function at 6 months after surgery in the treatment of traumatic nonunion of tibial and fibular fractures.However,LCP showed significant advantages in accelerating fracture healing,had a relatively lower risk of postoperative complications,and provided better early recovery of knee function.关键词
