开放性眼球损伤患者术后再次手术的危险因素分析及预测模型构建
Analysis of Risk Factors and Construction of Prediction Model for Reoperation in Patients with Open Globe Injury after Surgery

Abstract
Objective To analyze the early visual characteristics and independent predictive factors for reoperation in patients with open globe injury(OGI),construct and validate a column chart model,and provide a basis for postoperative risk stratification and intervention strategies.Method 200 OGI patients admitted from January 2022 to December 2024 were included,and their demographic and eye in-jury data were recorded.Multivariate logistic regression was used to screen for independent risk factors for reoperation,and a column chart prediction model was constructed.Calibration curves and ROC curves were used to evaluate the discrimination and calibration abil-ity of the model.Result Among the 200 patients,46 required reoperation.The initial visual acuity of the reoperation group before and af-ter surgery was significantly lower than that of the control group,and the postoperative visual acuity improvement rate of the control group was significantly higher than that of the reoperation group.Initial visual acuity≤0.1,male gender,iris prolapse,retinal detachment,local anesthesia,and BETT injury type are independent influencing factors for reoperation in patients with open eye injury.Based on this,a model was constructed to predict the incidence of reoperation in patients with OGI with an AUC of 0.849(95%CI:0.768-0.929),sensi-tivity of 0.679,and specificity of 0.871.The Hosmer Lemeshow goodness of fit test showed that the prediction model has good calibration ability(χ2=2.264,P=0.759).Conclusion The column chart model constructed in this study integrates key factors such as initial visual acuity,gender,iris prolapse,retinal detachment,anesthesia method,and BETT classification,which can effectively predict the risk of re-operation in OGI patients and assist in individualized clinical decision-making.关键词
