分隔式胃空肠吻合术与传统胃空肠吻合术治疗局部晚期胃癌幽门梗阻效果比较
Application analysis of stomach-partitioning gastrojejunostomy in the treatment of pyloric obstruction in locally advanced gastric cancer

Abstract
Objective To compare the therapeutic efficacy and prognostic outcomes of stomach-partitioning gas-trojejunostomy(SPGJ)and conventional gastrojejunostomy(CGJ)in patients with locally advanced gastric cancer compli-cated by pyloric obstruction.Methods A retrospective analysis was conducted on clinical data from 100 patients with lo-cally advanced gastric cancer and pyloric obstruction treated at the Department of Gastrointestinal Surgery,Affiliated Canc-er Hospital of Guangxi Medical University between January 2018 and September 2024.Thirty-eight patients underwent SPGJ(SPGJ group),and sixty-two underwent CGJ(CGJ group).Preoperative baseline characteristics,operative time,intraoperative blood loss,postoperative complications,recovery of bowel function,time to oral intake,nutritional indica-tors,and postoperative outcomes were compared between the two groups.Results Compared with the CGJ group,pa-tients in the SPGJ group demonstrated significantly faster recovery to full and semi-liquid diets(t=-2.787,P=0.007;t=-1.999,P=0.049),shorter times to first flatus/defecation and nasogastric tube removal(t=-2.446,P=0.016;t=-2.448,P=0.016),better postoperative 1-month weight maintenance(Z=-2.034,P=0.042),higher minimum levels of serum total protein and albumin within 1 month postoperatively(t=2.643,P=0.010;t=2.336,P=0.022),lower rates of reoperation due to complications(x2=3.912,P=0.048),and improved performance status scores(x2=17.481,P=0.002).Conclusion For patients with locally advanced gastric cancer complicated by pyloric obstruction,SPGJ offers significant advantages over CGJ and may be considered the preferred surgical option.关键词
