血浆CEA、CA125和CA153检测在乳腺癌诊断中的应用策略
Application strategy of plasma CEA,CA125,and CA153 in the diagnosis of breast cancer

Abstract
Objective To evaluate the clinical value of three tumor markers-carcinoembryonic antigen(CEA),cancer antigen 125(CA125),and cancer antigen 153(CA153)—in the screening and diagnosis of breast cancer.Meth-ods A retrospective analysis was conducted on 636 breast cancer patients,578 patients with benign breast lesions,and 651 healthy controls who visited Yuebei People's Hospital Affiliated to Shantou University Medical College between January 2020 and December 2021.The differences in the plasma levels of the three tumor markers among the three groups were compared,and the sensitivity and specificity of each marker were analyzed.Results Plasma levels of CEA,CA125,and CA153 were significantly higher in the breast cancer group compared to the benign and healthy control groups(P<0.001).Among the individual markers,CA153 demonstrated the highest sensitivity(63.99%),while CEA showed the highest specificity(76.65%).CA153 also had the largest area under the receiver operating characteristic(ROC)curve(AUC=0.629).Among the combinations,CEA+CA153 yielded the highest sensitivity(53.46%),CA125+CA153 showed the highest diagnostic accuracy(84.38%),and the combination of CEA+CA125+CA153 achieved the highest AUC(0.668).Higher levels of all three markers were observed in patients with larger tumors,advanced stages,and ex-tensive lymph node metastasis.Significant differences in marker expression were found among different molecular subtypes of breast cancer.Conclusion Among the tested tumor markers,the combination of CA125 and CA153 offers the highest diagnostic specificity,while CA153 alone provides the highest sensitivity for breast cancer diagnosis.Differential expres-sion of plasma CEA,CA125,and CA153 may assist in the diagnosis of various molecular subtypes of breast cancer.关键词
