潜伏期经腹部及会阴超声影像组学预测初产妇分娩结局的临床研究
Clinical research on predicting delivery outcomes of primiparous women using abdominal and perineal ultrasound imaging omics during incubation period
摘要
Abstract
Objective To explore the value of abdominal and perineal ultrasound imaging omics during incubation period in predicting delivery outcome of primiparous women.Methods A total of 400 primiparous women with full-term pregnancies who underwent vaginal trial labour at the Obstetrics Department of Jiangxi Provincial Maternal and Child Health Hospital from January 2023 to December 2024 were selected as the study subjects,they were divided into the natural delivery group(361 cases)and the cesarean section group(39 cases)according to the delivery method.Both groups underwent abdominal and perineal ultrasound examinations during the incubation period(cervical dilation of 2-6 cm).Imaging parameters includ-ing angle of progression(AOP),fetal head direction angle(HD),head-perineum distance(HPD),and midline angle(MLA)were measured.Ultrasound imaging parameters were analysed between the two groups,ROC curves were plotted to analyse the efficacy of imaging-based prediction of mode of delivery,and Cox models were used to analyse the prediction of delivery duration.Results The AOP in the natural delivery group was higher than that the caesarean section group,while HD,HPD,and MLA in the natural delivery group were lower than those the caesarean section group,with statistically significant differences(P＜0.05).The AUC for the combined prediction of caesarean section using the above parameters was 0.892.There were statistically significant differences in delivery duration between the AOP,HD,HPD,and MLA groups(P＜0.05).Cox model analysis showed that AOP,HPD,and MLA were independent factors predicting delivery duration(P＜0.05).Conclusion Incubation period abdominal and perineal ultrasound imaging can effectively predict the mode and duration of delivery,which has important clinical value.关键词
