ICU菌血症患者菌群的影响因素及诊断价值分析
Influencing factors and diagnostic value of bacterial flora in ICU patients with bacteremia

Abstract
Objective To explore the influencing factors and diagnostic value of the microbiota in patients with bacteremia in the ICU,and to provide a theoretical basis for establishing a rapid and reliable method for identifying pathogenic bacteria.Methods A retrospective analysis was conducted on the clinical data of 80 patients with bacteremia in the ICU admitted to Xinyu People's Hospital from January 2023 to December 2024.According to the characteristics of the infectious flora,they were divided into the gram-negative group(40 cases)and the gram-positive group(40 cases).The serum procalcitonin(PCT)levels of all patients were measured,and the relevant test data,case data,etc.were completed.The clinical data of the two groups of patients were compared to analyze the characteristics of the microbiota and the diagnostic value of microbiota types in ICU patients with bacteremia.Results The comparison of serum PCT levels between the two groups of patients showed a statistically significant difference(P<0.05).Serum PCT level(β=1.760,OR=5.815,95%CI:2.488-13.593)was an inde-pendent risk factor for gram-negative bacterial infection(P<0.05).The ROC curve showed that the AUC of serum PCT in differentiating the microbiota types of ICU patients with bacteremia was 0.957,which had a high discriminatory value,and the optimal cut-off value was 6.855 ng/ml.Conclusion Serum PCT concentration is related to the microbiota characteristics of ICU patients with bacteremia,which can effectively distinguish gram-negative bacteria and gram-positive bacteria,pro-viding an objective basis for clinical diagnosis and treatment.关键词
