超声引导菱形肌-肋间肌平面阻滞在乳腺切除术中的应用

Abstract
Objective To explore the anesthetic efficacy of ultrasound-guided rhomboid intercostal plane block(RIPB)in breast cancer patients undergoing mastectomy.Methods A total of 60 patients undergoing mastectomy admitted to Nanchang People's Hospital from January 2023 to June 2024 were selected as study subjects.They were divided into the control group(30 cases)and the observation group(30 cases)according to the random envelope method.The control group received total intravenous combined anterior serratus plane block,while the observation group received total intravenous combined RIPB.The onset time,duration of action,dosage,pain scores,incidence of adverse reactions,and number of analgesic pump presses within 24 hours were compared between the two groups.Results There were no statistically significant differences between the two groups in terms of the onset time of sensory blockade or duration of blockade(P＞0.05).There were no sta-tistically significant differences in the total consumption of Remifentanil,Propofol,and 24-hour Sufentanil between the two groups(P＞0.05).At 6,12,24,and 48 hours post-surgery,the visual analogue scale(VAS)scores in the observation group were lower than those in the control group,with statistically significant differences(P＜0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,with statistically significant difference(P＜0.05).The number of presses on the analgesic pump within 24 hours in the observation group was less than that in the control group,with statistically significant difference(P＜0.05).Conclusion In the management of patients undergoing mastectomy,ultrasound-guided RIPB anaesthesia achieves the desired anaesthetic effect with minimal adverse reactions,demonstrating high clinical utility.关键词
