参芪地黄加味汤治疗脾肾两虚、血瘀湿热型痛风性肾病的临床观察
Clinical Efficacy of Shenqi Dihuang Jiawei Decoction in the Treatment of Gouty Nephropathy with Spleen-kidney Deficiency and Blood Stasis and Dampness-heat Syndrome

Abstract
Objective:To observe the clinical efficacy and safety of Shenqi Dihuang Jiawei Decoction in treating gouty nephropathy(GN)with spleen-kidney deficiency and blood stasis and dampness-heat syndrome.Methods:A total of 120 GN patients with spleen-kidney deficiency and blood stasis dampness-heat syndrome,admitted to Chongqing Traditional Chinese Medicine Hospital from January 2023 to March 2024,were randomly assigned to a control group and an observation group(60 patients each)using a random number table.The control group received conventional Western medicine treatment,while the observation group received Shenqi Dihuang Jiawei Decoction in addition to the control treatment.The treatment course for both groups was 4 weeks.Urinary protein excretion,early renal injury markers,renal function,inflammatory mediators,TCM syndrome scores,clinical efficacy,and ad-verse reactions were compared between the two groups before and after treatment.Results:After treatment,the total effective rate in the observation group was 86.67％,higher than 61.66％in the control group(P＜0.05).The observation group had significantly lower levels of urinary protein excretion[urinary protein/creatinine ratio(UACR)],early renal injury markers[urinary α1-micro-globulin(α1MG),urinary transferrin(UTRF),N-acetyl-β-D-glucosaminidase(NAG)],renal function indicators[serum creatinine(SCr),blood urea nitrogen(BUN),uric acid(UA)],inflammatory mediators[high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)],and TCM syndrome scores compared with the control group(P＜0.05 or P＜0.01).The estimated glomerular filtration rate(eGFR)was significantly higher in the observation group than in the control group(P＜0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P＞0.05).Conclu-sion:Shenqi Dihuang Jiawei Decoction is clinically effective in treating GN with spleen-kidney deficiency and blood stasis and dampness-heat syndrome.It can significantly reduce proteinuria,improve renal function,delay disease progression,and its mecha-nism may be related to the inhibition of inflammatory responses,with good safety.关键词
