Abstract

Objective:To investigate the mechanism of action of Juanbi Capsules in treating a rat model of knee osteoarthritis(KOA)via the phosphatidylinositol 3-kinase(PI3 K)/protein kinase B(AKT)signaling pathway.Methods:Sixty rats were ran-domly divided into six groups,with 10 rats per group.The blank group was fed normally,while the other groups underwent KOA modeling.Three rats were randomly selected six weeks after modeling to confirm successful model establishment.After modeling,the model group was fed normally,while treatment groups received gavage administration.Samples were collected after four weeks of ga-vage.ELISA was used to measure matrix metalloproteinase-13(MMP-13),a disintegrin and metalloproteinase with thrombospondin motifs-5(ADAMTS-5),interleukin-10(IL-10),and interleukin-1β(IL-1β).Protein expression of runt-related transcription factor 2(RUNX-2),SRY-box transcription factor 9(SOX9),and type Ⅱ collagen(COL-Ⅱ)in cartilage was detected by Western blot.Histopathological changes were assessed using hematoxylin-eosin(HE)staining,and bone microstructure was evaluated by micro-CT.Results:ELISA results showed that levels of MMP-13,ADAMTS-5,and IL-1β in the blank group were lower than those in other groups,and within the treatment groups,the low-dose group had higher levels than the medium-and high-dose groups(P<0.05).The model group had lower levels than all treatment groups(P<0.05).Micro-CT analysis revealed increased bone microstructure expression in the high-,medium-,and low-dose groups compared with the model group(P<0.05).Compared with the Western medicine group,the low-dose group had lower bone microstructure expression(P<0.05).HE staining showed severe cartilage dam-age in the model group,with obvious thinning and loss of cartilage and fissures extending into the subchondral bone.Low-,medi-um-,and high-dose treatment groups,as well as the Western medicine group,exhibited only mild cartilage defects,with largely in-tact subchondral bone.Western blot indicated that RUNX-2,COL-Ⅱ,and SOX9 protein expression in the low-and medium-dose groups was lower than that in the high-dose group(P<0.05).Conclusion:Juanbi Capsules promote the expression of FOXO via the PI3K/AKT pathway,which enhance RUNX-2,COL-Ⅱ,and SOX9 expression and reduce inflammation.Among the doses test-ed,the high dose may be the optimal therapeutic dose for KOA.
