Abstract

Objective Based on the"phlegm-stasis generating wind"theory in traditional Chinese medicine(TCM),this study aimed to investigate the mechanism of Qutan Huayu Herbal Formula(祛痰化瘀中药复方,QHHF)in atrial fibrillation(AF)via the ferropto-sis pathway.Methods A total of 120 healthy SD rats were randomly assigned to the following groups:the blank control,model,low/medium/high-dose QHHF,and Western medicine(amiodarone)groups.The blank control group received 0.9%NaCl solution via tail vein injection,whereas the other groups were injected with acetylcholine-CaCl2(ACh-CaCl2)to induce AF.After successful modeling,rats in the QHHF groups were administered the corresponding QHHF solution at low/medium/high doses via gavage,while the Western medicine(WM)group received amiodarone hydrochloride,for a continuous period of two weeks.The following assessments were con-ducted after treatment:electrocardiogram(ECG)was used to measure AF duration;HE staining was employed to observe atrial tissue pathology;transmission electron microscopy(TEM)was utilized to examine ultrastructural changes;commercial assay kits were used to measure the levels of GSH,ROS,MDA,and SOD;quantitative real-time polymerase chain reaction(qRT-PCR)and Western blot(WB)were performed to determine the mRNA and protein expression levels of p53,SLC7A11,GPX4,TFR1,FTH1,and DMT1 in the atrial tissue.Results Compared with the model group,the low/medium/high-dose QHHF groups and WM group showed a significant reduction in AF duration(P<0.05 or P<0.01).The levels of GSH and SOD were significantly higher,while the levels of ROS and MDA were significantly lower,in all treatment groups compared to the model group(all P<0.05 or P<0.01).The mRNA and protein expression levels of p53,TFR1,and DMT1 were significantly downregulated,whereas those of SLC7A11,GPX4,and FTH1 were sig-nificantly upregulated,in the atrial tissue of rats from all treatment groups compared to the model group(all P<0.05 or P<0.01).Con-clusion The mechanism underlying ferroptosis-induced AF is consistent with the TCM theory of"phlegm-stasis generating wind"leading to palpitations.Iron overload and lipid peroxidation products may represent the material correlates of TCM phlegm-turbidity and blood-stasis.The QHHF formula likely alleviates AF by modulating the ferroptosis pathway.
