Effect of stepped visual function training on binocular vision rehabilitation in children with concomitant exotropia after surgery

Abstract
Objective To investigate the efficacy of stepped visual function training in binocular vision rehabilitation and identify key influencing factors in children with concomitant exotropia after strabismus surgery.Methods This prospec-tive study enrolled 126 children diagnosed with concomitant exotropia who underwent strabismus surgery at the Ophthalmol-ogy Department of Xiangya Hospital,Central South University,between July 2024 and October 2024.Participants were randomly assigned to either accommodative target convergence training(n=59)or stepped visual function training(n=67).Postoperative alignment control and binocular vision function(assessed via synoptophore)were compared at 1,3,and 6 months.Statistical analyses were performed.Results At 6 months postoperatively,the orthophoria rates were 89.83%(53/59)in the accommodative training group and 91.04%(61/67)in the stepped training group,with no significant intergroup differences in deviation angles at any timepoint(all P>0.05).No differences were observed in simultaneous perception coincidence points or divergent fusion at 1,3,and 6 months(all P>0.05).At 1 month postoperatively,there were no statistically significant differences between the two groups in convergent fusion range,or distance stereopsis(P>0.05).However,at 3 and 6 months postoperatively,the stepped training group demonstrated significantly superior convergent fusion,fusion range,and distance stereopsis at 3 and 6 months(all P<0.05).Efficacy evaluation based on 6-month distance stereopsis acuity revealed significantly better outcomes in the stepped training group(P<0.05).GBDT modeling identified age(76.49%),training protocol(38.70%),and preoperative fusion function(16.41%)as core predictive factors.Conclusion Stepped visual function training effectively enhances high-grade binocular vision rehabilitation in children with concomitant exotropi-a.Age,training protocol,and preoperative fusion function are critical determinants of postoperative stereopsis recovery,underscoring the need for stratified rehabilitation strategies tailored to these parameters.关键词
