玉树高原地区小学生视力筛查结果分析以及轴率比对近视的诊断试验评价
Analysis of vision screening results and evaluation of the diagnostic test for myopia based on axial length/corneal radius ratio in primary school students in the yushu plateau area

Abstract
Objective To analyze the characteristics of visual acuity and ocular biometric parameters among primary school students in the Yushu plateau area,and to evaluate the diagnostic efficacy of the axial length/corneal radius ratio(AL/CR)for myopia.This study aims to provide evidence for myopia prevention and control among children in high-alti-tude regions of China.Methods A cross-sectional study was conducted in September 2024 among randomly selected students in grades 1 through 6 from three schools in the Yushu Tibetan Autonomous Prefecture,Qinghai Province,using a direct sampling approach.Parameters such as uncorrected visual acuity,axial length(AL),mean keratometry(K),AL/CR,and spherical equivalent(SE)after pupil dilation were measured.Results This study included 955 children aged 7~12 years(10.00±1.08 years),with boys accounting for 467(934 eyes,48.9%)and girls accounting for 488(976 eyes,51.1%).The proportion of normal vision was 58.28%,with the highest proportion of normal vision observed at age 8(72.8%).Among children aged 9~12 years,the proportion of normal vision showed a decreasing trend with increasing age(from 67.97%to 42.24%).The mean values of AL,K,and AL/CR in the overall population were:22.93±0.98 mm,43.58±2.17 D,and 2.96±0.11,respectively.AL/CR ≥3 had a sensitivity of 84.3%and specificity of 90.4%for diagnosing myopia,with a Youden index of 0.59 and an area under the ROC curve of 0.863.Its diagnostic efficacy for myopia was relatively high in children aged 9~12 years(area under the ROC curve of 0.867~0.87),but lower in children aged 7 years(area under the ROC curve of 0.625).Conclusion The proportion of normal vision among primary school students in the Yushu plateau area is relatively high,but it shows a decreasing trend with increasing age.The diagnostic performance of AL/CR≥3 is effective for detecting myopia.关键词
