Observational Study on Changes in Astigmatism after Wearing Differently Designed Myopic Defocus Spectacle Lenses

Abstract
Objective To explore the changes in astigmatism in children and adolescents after wearing two different designs of multi-focal myopia defocus lenses:Defocus Incorporated Multiple Segments(DIMS)and Highly Aspherical Lenslets(HAL).Methods Children aged 7～12 years who visited the Optometry Center of Beijing Aier Yingzhi Eye Hos-pital from September 2020 to March 2021 were selected and divided into three groups:54 children wearing DIMS,60 children wearing HAL,and 48 children wearing single-vision lenses(SVL).The changes in total ocular astigmatism and corneal astigmatism were observed in children and adolescents after wearing the three differently designed lenses for 6 months and 12 months without other interventions.Results After 6 months of wear DIMS group:total astigmatism increased by 0.14±0.38D,corneal astigmatism increased by 0.09±0.24D.HAL group:total astigmatism increased by 0.06±0.08D,corneal astigmatism increased by 0.15±0.32D.SVL group:total astigmatism increased by 0.24±0.14 D,corneal astigmatism increased by 0.10±0.30D.The increase in total astigmatism in the SVL group was significantly higher than in both the DIMS and HAL groups,and this difference was statistically significant(P＜0.01).No statistically signif-icant difference was observed in corneal astigmatism changes among the three groups(P＞0.05).After 12 months of wear DIMS group:total astigmatism increased by 0.24±0.17D,corneal astigmatism increased by 0.15±0.25D.HAL group:total astigmatism increased by 0.16±0.17D,corneal astigmatism increased by 0.15±0.36D.SVL group:total astigmatism increased by 0.48±0.25D,corneal astigmatism increased by 0.20±0.35D.The increase in total astigmatism in the SVL group was significantly higher than in both the DIMS and HAL groups,and this difference was statistically significant(P＜0.01).No statistically significant difference was observed in corneal astigmatism changes among the three groups(P＞0.05).Conclusion Compared with single-vision lenses,the changes in total ocular astigmatism diopter in children and adolescents after wearing myopia defocus lenses designed with Defocus Incorporated Multiple Segments and Highly Aspherical Lenslets are smaller,and there is no signifi-cant impact on the changes in corneal astigmatism.This provides a clinical basis for the personalized selection of myopia prevention and control programs for children and adolescents.关键词
