Influence of ICF Framework Individualized Rehabilitation Regimen under the Concept of Enhanced Recovery after Surgery on Knee Cruciate Ligament Reconstruction

Abstract

Objective:To investigate the influence of international classification of functioning,disability and health(ICF)framework individualized rehabilitation regimen under the concept of enhanced recovery after surgery(ERAS)after knee cruciate ligament reconstruction.Method:From March 2023 to September 2024,80 patients after knee cruciate ligament reconstruction in Department of Rehabilitation Medicine of the First Affiliated Hospital of Hainan Medical University were selected as the research subjects.According to the random number table method,they were divided into control group and experimental group,with 40 cases in each group.The control group received routine rehabilitation therapy,while the experimental group was treated with ICF framework individualized rehabilitation regimen under the concept of ERAS.The surface electromyography indicators,function of the affected knee joint[active range of motion(AROM)of knee flexion and extension,Chinese version of knee injury and osteoarthritis outcome score(KOOS)],pain degree[numerical rating scale(NRS)score],anxiety status(SAS),quality of life(SF-36),and return to work ability[19-item return-to-work self-efficacy scale(RTWSE-19)]were compared between groups before and after intervention.Result:After intervention,the myoelectric signal values of quadriceps femoris contraction and relaxation and hamstring muscle contraction and relaxation,AROM of knee flexion and extension and KOOS dimension scores of the two groups were all higher than those before the intervention,and the indicators in experimental group were higher(P<0.05).Repeated measures analysis of variance showed that there were time,between-group and interaction effects in the changes of NRS scores at different time points after intervention in the two groups(Ftime=13.744,Fbetween-group=6.418,Finteraction=9.264,P<0.001).There were time,between-group and interaction effects in the changes of SAS scores at different time points after intervention in the two groups(Ftime=16.283,Fbetween-group=9.529,Finteraction=12.647,P<0.001).Within 6 weeks of intervention,the NRS scores and SAS scores of both groups gradually decreased,the NRS scores of the experimental group were lower than those of the control group in the 2nd and 4th weeks of intervention,and the SAS scores of the experimental group were lower than those of the control group in the 2nd,4th,and 6th weeks of intervention(P<0.05).After intervention,the scores of each dimension of SF-36 and RTWSE-19 scale in the experimental group were higher than those in control group(P<0.05).Conclusion:ICF framework individualized rehabilitation regimen based on the concept of ERAS can effectively promote the muscle function and joint range of motion recovery in patients after knee cruciate ligament reconstruction,improve the pain and psychological health status,and promote the quality of life and return to work ability.
