肥胖伴与不伴睡眠呼吸暂停患者袖状胃切除术的疗效比较
Comparative effectiveness of sleeve gastrectomy in obese patients with and without obstructive sleep apnea syndrome

Abstract
Objective:To compare and analyze the weight loss outcomes and metabolic disease improvement in obese patients with obstructive sleep apnea syndrome(OSAS)versus those without OSAS who underwent laparoscopic sleeve gastrectomy.Methods:The clinical data of patients who underwent laparoscopic sleeve gastrectomy from Jan.2022 to May 2023,along with their 1-year post-operative follow-up data,were analyzed.According to polysomnography monitoring results,patients were divided into an obese with OSAS group(observation group)and a non-OSAS group(control group).Anthropometric parameters(body mass index,weight,three circumferences),body fat percentage,apnea-hypopnea index and oxygen saturation were compared between the two groups at preopera-tive,6-month,and 12-month postoperative time points.Results:A total of 72 obese patients were included,with 47 in the observation group and 25 in the control group.There were 42 males and 30 females,with a mean age of(33.86±7.97)years and a mean BMI of(43.27±6.75)kg/m2.In the observation group,the minimum nocturnal oxygen saturation was 83.64％±4.67％at 6 months and 91.19％±2.69％at 1 year postoperatively(P＜0.05).At 6 months postoperatively,the three circumferences showed statistically signi-ficant differences between the two groups(P＜0.05).All three circumferences significantly decreased in both groups at 6 months post-operatively(P＜0.05).The control group showed progressive decreases in weight,BMI,and three circumferences at 12 months,which were superior to the observation group(P＜0.05).There was a significant difference in the reduction of body fat percentage between the two groups(P＜0.05).Comparative results showed that the control group had significantly higher postoperative weight benefits(P＜0.05).Conclusions:Although the weight loss effect of laparoscopic sleeve gastrectomy is less pronounced in obese patients with OSAS compared to those without OSAS,it can significantly alleviate OSAS symptoms,thereby offering dual clinical benefits.关键词
