Abstract
Objective:To investigate the safety and feasibility of emergency laparoscopic cholecystectomy(LC)for the treat-ment of acute calculous cholecystitis(ACC)in elderly patients.Methods:Clinical data of 92 patients with ACC who underwent emer-gency LC from Jun.2018 to Oct.2024 were retrospectively analyzed.Patients were divided into an elderly group(n=45)and a non-elderly group(n=47)based on age.Preoperative general data,intraoperative findings,and postoperative outcomes were compared be-tween the two groups.Results:The elderly group had higher age,proportion of patients with ASA grade ≥2,and overall incidence of preoperative comorbidities compared to the non-elderly group(P<0.05).No statistically significant differences were found in other baseline clinical data(P>0.05).There were no deaths in either group.One case in each group was converted to open surgery due to in-traoperative massive bleeding,and one additional case in the elderly group was converted due to severe adhesions.Postoperative compli-cations occurred in 8 cases.Incision infections in 3 patients(2 in the elderly group,1 in the non-elderly group)were controlled within 5 d with intensified wound care.Bile leakage in 3 patients(2 in the elderly group,l in the non-elderly group)resolved with symptoma-tic treatment within 7~10 d,without the need for reoperation.Two cases of pneumonia in the elderly group were cured with anti-infec-tive therapy.No statistically significant differences were found between the two groups in intraoperative blood loss,operative time,con-version rate,time to drainage tube removal,time to postoperative flatus,overall postoperative complication rate,postoperative hospital stay,total hospital stay,hospitalization costs,or pathological classification(P>0.05).Conclusions:Emergency LC is a safe and feasible treatment for elderly patients with mild to moderate ACC.关键词
